The Intake Forms (see above) require Adobe Reader to view it. If you do not already have Adobe Reader on your computer you can download it for free. Just be sure you ask for the correct operating system for your computer (e.g. Windows vs. Mac) - Adobe Reader X / Free Download Here .
Once you have downloaded this application, open it. Then click on the Intake Form file (see above) to download the file. You can either print out the six page Intake Form and manually complete it or you can fill out the Form right on your computer. Instructions for completing Intake Forms are found below on Download 2 (see below).
