- Complete Legal Name
- Address
- Contact Phone Number(s)
- Insurance Company and Type of Plan (PPO / EPO / HMO / etc) or N/A - non-applicable
- If a minor, or you are under your parent's insurance plan, please include insured Parent's full legal name and address
I will then email you back a zip file that will include:
Office Policies / Instructions for Completing Intake Forms
- General Office Policies including Cancellation Policy (48 hrs), forms of payments taken, Instructions if you need Insurance reimbursement
- General Directions to my location and Instructions on what to do when you arrive.
- Instructions for completing the last page of your Intake Forms.
- Please be sure to read before completing the Intake Forms
Intake Forms / Consent Forms / Release Forms
The Intake Forms require EXCEL (.xlsx) program to view it. You can either print out the Intake Form and manually complete it or you can fill out the Form right on your computer.
Also, please read and sign Informed Consent Forms. You may bring completed forms with you to your first appointment, and/or e-mail them directly back ahead of time to
